How to Submit a Claim

Submitting a claim with your Group Fixed Indemnity Plan is simple—and similar to how traditional health insurance works. Here’s a step-by-step guide:

Visit your doctor

Show your Ease Total Health ID card at your appointment, just like you would with regular insurance.

2.

Pay any required amount up front

Your provider may ask for a partial payment at the time of service.

After your visit, you’ll receive a medical bill by mail.

3.

Wait for your bill

File your claim through the Ease Total Health App:

  • Open the app and go to “My Benefits”

  • Tap on the “Benefits” tab

  • Select “File a Claim”

  • Upload a photo of your bill directly from your phone

5.

Get paid

Once your claim is reviewed and approved, you'll receive a cash payment.

Coming Soon: You’ll be able to access and manage your payment through the new “My Pay” section in the app.

That’s it! The process puts you in control and the cash is sent directly to you.
